I really hurt my Shoulder?

Ok so last night i stayed at my friends house and i was standing on her couch trying to get something but then i fell and hit my right shoulder of off her fireplace and it hurts so bad, also i have had previous problems with this shoulder and pulling a muscle but nothing ever hurt this bad please help easy points. ;)
